What the character needs first

The secret is gated in the game. It asks for Appreciative standing with Ve'nari, the broker of the Maw, and the character must be able to enter both the Beastwarrens and Perdition Hold; our order lists the Appreciative reputation with Ve'nari as well. The character also has to be at max level; if yours is not there yet, our leveling carry can take it there first.

The secret, step by step

Buy the Stygia Dowser from Ve'nari, paid in Stygia.
Use the dowser on Stygia Nexus nodes and drain them; they give Stygia Sliver and Stygia Dust.
In Perdition Hold, on the Altar of Domination, solve the rune puzzle of the Runed Chest in four swaps. The Crumbling Stele is your reward.
Get the Partial Rune Codex from Ve'nari, again for Stygia, and track down its pages for souls, forging and binding.
Go to Cocyrus and finish Feral Shadehound, the quest at the Binding Altar. The hound is learned.

A hound bound to its maker

The journal tells how this shadehound was created by decoding and casting strong runes, and it remains tied to whoever made it. Few secrets in the Maw take longer, and every part of the ritual has to be played through. The hound itself is a ground mount.

Its last step is Feral Shadehound, a quest at the Binding Altar of Cocyrus. First you solve the rune puzzle in Perdition Hold and collect all three codex pages.

Which reputation does it involve?

Appreciative with Ve'nari. She is also the vendor for two items the secret needs: the Partial Rune Codex and the Stygia Dowser.

How does the Runed Chest puzzle work?

You find it in Perdition Hold, on the Altar of Domination. Swap the runes four times and it opens, giving you the Crumbling Stele.
